Free help is available

If you have a disability, we provide free aids and services to help you communicate with us effectively, such as qualified sign language interpreters and written information in other formats, including large print. Tell the office what you need when you schedule and we will arrange it at no cost to you.

If English is not your primary language, we provide free language assistance services, including qualified interpreters and information written in other languages.

If you need any of these services, call the office at (502) 893-7151. There is never a charge to you.

If you believe we have failed to provide these services, or have discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with:
